01 / BRING YOUR PDF

Start with the file you already use.

A PDF is a document that keeps its page layout when you share it. A permission slip, waiver, or signup sheet is a good place to start.

  1. Create an account or sign in. This is your space for managing forms.
  2. Choose a PDF or drop it into the upload box. The file can be up to 25 MB.
  3. Choose Create my form. AI suggests the questions and where the answers should go.

If automatic setup fails, your uploaded PDF stays saved. Choose Try AI setup again or Set up manually.

02 / CHECK AND TRY IT

You get the final say.

AI makes suggestions. Check the sample PDF and questions before you send the form to anyone.

Field
A place for an answer, like “Child’s name” or a signature.
Required
A question someone must answer before submitting.
Optional
A question someone can leave blank. These appear in a separate expandable section.
Shared answer
An answer you set once for everyone, like the trip date or school name. Respondents don’t need to type it.
  1. Open Sample PDF. Make sure answers fit in the right places on every page.
  2. Use Shared answers or Fields & placement to make changes. Choose Save & refresh preview when you’re done.
  3. Choose Try filling it out for a practice run. You can also scan the preview’s QR code on another device.
How do I use one answer in several places?

Open Fields & placement, open a field, and choose another field under Use the same answer as. For example, link every place that asks for the same parent’s name. Respondents answer once, and that answer fills all linked locations. Only matching answer types can be linked; shared answers link to other shared answers.

For shared answers, the selected field’s value is used everywhere in the group. Choose Separate answer (not linked) to unlink one location; it keeps a copy of the shared value. Changes to the answer type or who fills it apply to the linked group. Choose Save & refresh preview when you’re done.

03 / PUBLISH AND SHARE

Give people a link to fill out.

Publish means making your form available to anyone with its public link. People filling it out don’t need an account.

  1. Choose Publish form to open the sharing settings. Opening these settings doesn’t send anything.
  2. Enter your sending email, its app password, and the address that should receive the link. Common email server settings are suggested for you.
  3. Choose Publish & email link. The app checks the email connection, sends the link, and marks your form published.
  4. Use Open published form to open the live form and copy its address to share.
What is an email app password?

Some email providers let you create a special password for apps that send mail. You get it from your email provider’s security settings. It may be different from your normal email password. “SMTP” is simply the connection the app uses to send those emails.

Why can’t someone else open my link?

The app needs to be running at an address they can reach. A link containing “localhost” only points to the computer opening it. For a local phone test, use the preview QR code on the same Wi-Fi and follow its connection settings. For wider sharing, use an app hosted at a public address. Publishing a form doesn’t host the app for you.

If the first email fails, a new form stays unpublished. Fix the email settings and try again.

04 / SEE THE RESPONSES

Your completed forms have a home.

A response is one person’s submitted set of answers. The app saves those answers and a completed PDF inside that form’s collection.

  1. Sign in and find the form under Your forms.
  2. Choose View responses. You’ll see the newest submissions first, with answer summaries and email delivery status.
  3. Choose Open PDF to see a completed document, including its signature.
  4. Choose Open CSV for a file you can open in Excel, Google Sheets, or another spreadsheet app.

If a submission email fails, the response is still saved. You can open its PDF here and see the email error in the list.

JUST FILLING OUT A FORM?

Open. Answer. Review. Send.

  1. Open the link from the organizer. You don’t need to create an account.
  2. Answer the required questions and draw your signature where asked. Add optional details if they apply.
  3. Choose Review completed PDF. Scroll through every page, then confirm that you’ve read it and your answers are correct.
  4. Choose Submit form and wait for the saved confirmation. If you edit your answers, review the PDF again before submitting.

Reviewing the PDF doesn’t submit it. The app doesn’t automatically email a separate copy to you; contact the organizer if you need one.

The little things, explained.

Will AI answer people’s questions for them?

No. AI suggests the form setup and shared answers. Each person provides their own personal answers and signature.

Do I need to type the same answer on every page?

If the owner has linked those fields, you answer once and the app fills all their locations in the PDF. Separate people’s answers should stay separate, so check this in your practice run.

What’s the difference between a draft and a ready form?

A draft is still being set up. A ready form has fields prepared for you to check. Neither is public until you publish it.

What happens if I publish again?

The app keeps the same public link and sends another invitation email using your publishing settings.

Can I come back to this guide later?

Yes. Choose Simple guide in the app or on the responses page. It opens in a separate tab from those pages, so you can keep your place.
